Welcome to the Ed White Clinic Please feel free to come by and meet our nurse, Patty Alvarado RN, and our clinic assistant, Irma Vasquez. We are here Monday through Friday 8:00 am - 4:00 pm Phone number: 650-1400 ext. 250 (clinic) 650-1400 ext. 246 (nurse) Fax number: 650-1415 SHOTS NO SHOTS, NO SCHOOL, NO KIDDING! Texas State law requires all students be up to date on their immunizations in order to attend school. Please take this matter seriously and provide documentation of the vaccines or your child will be excluded from school on the specified compliance dates. (Click on the "resources" link at the left for a list of Health Department immunization clinics) MEDICATION (IMPORTANT... PLEASE READ CAREFULLY) NEIDS has a very strict medication policy. Students who do not follow this policy are referred to an dministrator. Please read in the student/parent handbook...the section on Health Services. There are strict consequences for students bringing medications to school themselves. NEISD policy on medications is as follows: Students are not to bring medications to school themselves. A parent or guardian must bring in the medications. Medications must be provided in the original container. Ziplock baggies, papertowels, etc. are not allowed. Medications for short term use will be accepted with a parent note for 10 calendar days only. After that, medications must be picked up by a parent, or will be discarded. If medications are needed longer than 10 days, a doctor's order must be provided. Most pharmacies are more than willing to label a second "school bottle" when filling prescriptions. Long term medications, and those needing to stay at school year round (ie. emergency medications, diabetic supplies, inhalers, etc.) must have a medication form signed by a doctor. Medications will not be accepted without proper documentation. Medication Administration Request Forms can be downloaded from the NEISD Health Services website (see links below), or you may pick up one from the school clinic. ILLNESS Students are expected to be free of fever and/or communicable disease in order to attend school. Parents/Guardians are required to pick up their students as soon as possible if the student develops a fever at school or exhibits other symptoms of illness, injury, or contagious disease, such as, but not limited to, rash, pinkeye, vomiting, diarrhea, or live head lice. Students may not be given medication for fever in order to keep them in school. The student is to be excluded from school until proper treatment has been administered and the condition has improved. A physician's statement may be required before a student is allowed to return to school. Blue Emergency Cards: Please turn these in completed and as soon as you receive them...By the 2nd day of school. Your signature gives us permission to treat your child in an emergency. It is very important that we have current/working phone numbers in case we have to reach you quickly. Please update the clinic as needed with new phone numbers. Your child's safety and well being are our concern. STUDENTS WILL NOT RELEASED TO ANY PERSON WHO IS NOT LISTED ON THE EMERGENCY CARD. Special Needs If your child has special medical needs or there is something you think the nurse should be aware of, please send a note or phone any time. I will share the information with teachers and staff on a need to know basis only. Please alert me to any allergies your child may have. We want to make school as safe as possible for your child. CATCH (Coordinated Approach To Children's Health) CATCH is a coordinated school health program which builds an alliance of parents, teachers, child nutrition personnel, school staff, and community partners to teach children and their families how to be healthy for a lifetime. www.catchtexas.org (CATCH website) to view the parent newsletters go to: http://www.sph.uth.tmc.edu/catch/CATCH_MiddleSchoolParentNewsletter.htm SCREENING Vision, hearing, and acanthosis screening will be done on all 7th graders starting in September.
